Review custom form submissions
This page explains how to review submissions of custom forms.
If you do not have the Review Queues tile, contact your TMV Client Success team.
In your Verivax admin/clinical portal go to the Review Queues tile:

Go to Custom Forms.
Note: Only form submissions with the Pending status will show by default.Click Review Case to view the form submission:

Available actions when viewing a form submission:
Click the Status to change the status of the form submission.
Enter a Case Note and click Submit to save the note.
Click Send Communication to send the user an email about their form submission.
You must set up a Form communication template first. Follow these instructions to create a custom communication.
You can edit the message content before sending the custom communication to the user.
Return a form submission to the In Progress status
If a form submission needs to be revised, you can return the form submission to the In Progress status. A common reason to do this is when a piece of documentation is missing for the form submission.
To return a submitted form to the In Progress status:
In your clinical/admin portal, go to Review Queues:

Go to the Custom Forms queue.
Search for the participant that the form is for.
Click View Case for the form submission.
Click the Status and select In Progress.
Click Save for the form submission.
After saving, you will see an option to open the form submission in the In Progress Forms Queue. We recommend following this to open the form submission in the In Progress Forms Queue.
After making the intended changes, submit the form. After submitting the form, return to the Custom Forms Review Queue to view the form submission with the updated answers.
If the form contains conditions, the conditions will be evaluated when the form is resubmitted. This means that if an answer is changed to an answer that satisfies a condition, the condition will execute the action when the form is resubmitted.
However, if a condition executed an action upon the original submission (e.g., added the user to a group), that action will not be undone if the answer is changed. In the case of a group assignment, the clinician would need to remove the user from the group.
