View a patient’s completed Immunization Surveys and Consents

Immunization Surveys and Consents are available to view while completing vaccine documentation for a patient.

Additionally, you can view any completed vaccine consents in Search for Patient before beginning vaccine administration.

To view a patient’s completed Immunization Surveys and Consents:

1. In your clinical role, go to Search for Patient.

2. Search for the patient.

3. Click Consents.

4. Completed consents will be hyperlinked in blue text with a checkmark. Click the consent you want to review.

Void a vaccine consent

Any vaccine consent that has not been tied to a vaccine patient encounter can be voided.

To void a vaccine consent:

1. Follow the instructions above to open the completed consent.

2. At the bottom of the consent survey, click Void and Start New Consent.

3. Confirm to compelte.

Complete new vaccine survey consent with a patient

If the patient has not completed an Immunization Survey and Consent, that consent will be hyperlinked in red text in the same list. Click the vaccine consent name to open and complete the vaccine survey consent with the patient.
