If you take a test, such as a COVID-19 test, you can submit results on TrackMy to share with your organization.
Submit a test result

In your TrackMy portal, click Submit Test Result. You'll find this button on your homepage, and also on the Tests page.

Select one or more test types you want to submit results for.

For each test, enter the requested information and click Next to move to the next test. You can click Go Back at any point to review or change what you already entered — your progress is saved as you move between tests in the same session.

Click Submit once you've completed every test.
Not every test will ask you to select a result — for some test types, your organization only needs you to upload documentation, and the result will be documented during review.
If you close or cancel out of this form before clicking Submit, nothing you entered is saved. You'll need to start over from the beginning.
Depending on your test result, your organization may ask you to complete more forms on TrackMy.
Image files and PDFs cannot be combined when uploading test results.
After you upload an image, you can upload additional images, but not PDF files.
If you start by uploading a PDF, you will not be able to upload any additional files.
View your test history
In your TrackMy portal, go to the Tests tile.
This opens your Test History page, showing every test result on file for you, organized by test category. Selecting a result shows more detail, including any file that was uploaded with it.
At this time, self-reported test results that have not yet been reviewed will not display on the Test Result History page.
If a result includes a normal reference range from the lab that performed it, values outside that range will display in color to help you spot them. This only applies to results received electronically from a lab your organization works with directly — it does not apply to tests you self report.
