Download user documents

The following user documents are available to download in Forms and Records:

• Consents

• Immunization surveys

• Administered vaccine records

• Self-reported vaccine records

• Complete vaccination records

• Accommodations and declinations

• OSHA medical evaluations

• Fit tests

• Exposure and symptom surveys

• Lab tests

To access user documents:

1. In the TrackMy admin/clinical portal, click the User Management tile:

2. Search for the user.

3. Click View Forms for the user. This button will not display if your account does not have permission to access user documents.

4. Click + to view documents in a category.

5. Click Download for the desired document.
