Submit a Tuberculosis survey

Your organization may require you to complete a Tuberculosis (TB) survey for risk assessment. This guide explains how to submit a Tuberculosis survey.

To complete the TB survey:

  1. In your TrackMy portal, click the Forms and Consents tile:

  1. Go to Survey and Immunization Consents and Start for the Tuberculosis Survey:

  1. Complete the survey and Save.

Your organization will review and approve your TB survey submission.
